Start with Washington County Environmental Health. It receives all county septic permit applications. Use its portal for new-construction, alteration, and repair permits. The portal also tracks applications and searches permits. You can schedule an inspection and see its result there. For an as-built, follow the county link to the GovQA public-records portal.
Do not combine the approval steps. A site evaluation is not permission to install. You need a separate construction-installation permit. It is generally valid for one year. A change in use or expected sewage flow can trigger another step. That step is an authorization notice.
